Enterprise Library Logging Http Module IIS 6.0

I am trying to create an http module that hooks into Enterprise Library logging to log application errors in Web 2.0 applications. Everything works fine (the errors are logged to the Logging database and an email is sent out) on my local machine; however, during deployment it does not log the errors nor send the email. Here is my code.

I have all the references in the /bin folder (not on the GAC). Is it even possible to use Enterprise Library Logging on IIS 6.0 in an Http Module? I have created several projects and none of them are working when deployed to the web server.
